How to write concise and clear website test documents?

There are several key elements to document every defect found after website testing:

a. Page URL link, e.g. http://www.paisuo.com/caseshow.html
b. Specific error on the page, such as incorrect text or misaligned images.
c. Standard correct content for reference, e.g. the correct text should be bb instead of xx.
d. Screenshot of the page content (optional).

Each test defect only needs to record the above three core elements a, b and c in the test document for straightforward review. Item d is optional.
